Parenting Plan forms
- In any case involving parenting time with children, both parties will need to file their proposed "Parenting Plan." The necessary forms are posted on the web site of the WV Supreme Court. You will need the Propsed Parenting Plan Instructions (4 pages); the Proposed Parenting Plan form (12 pages); the Parenting Plan Worksheet (4 pages); and the Motion to Adopt Proposed Parenting Plan (1 page).
