Forms for Filing a Divorce
- The forms that may be required in a divorce case are posted the web site of the WV Supreme Court. To get started, you will need the Instructions (16 pages); the form Petition (7 pages); the Verfication (1 page); and the Financial Statement (9 pages). Depending on your case, other forms may be needed as well. Read the Instructions carefully.

Forms for Responding to a Divorce
- The forms that may be needed to respond to a divorce filed against you are posted on the web site of the WV Supreme Court. To get started you will need the Answer Instructions (13 pages); the Answer form (5 pages); and the Financial Statement (9 pages). Depending on your case, other forms may be required. Read the Instructions carefully.

Parenting Plan forms
- In any case involving parenting time with children, both parties will need to file their proposed "Parenting Plan." The necessary forms are posted on the web site of the WV Supreme Court. You will need the Propsed Parenting Plan Instructions (4 pages); the Proposed Parenting Plan form (12 pages); the Parenting Plan Worksheet (4 pages); and the Motion to Adopt Proposed Parenting Plan (1 page).
